How to fill MoneyGram money transfer form

MoneyGram “Send Money” form may varies between countries. This form comes from Malaysia, which I got it from Maybank Foreign Exchange Booth here in Kota Kinabalu. There may be slight differences, but basically, I would like to highlight few important things.

For easy presentation, I have edited the form (yes, using GIMP) and labeled it according to the sections. Please read and fill the form properly. Otherwise we may not be able to cash your money out, here in Kota Kinabalu. And it will delay the necessary payment for securing your accommodation in Laban Rata.

Sections:

• {A} DESTINATION: Please fill in “MALAYSIA” for Country, and “KOTA KINABALU” for City.
• {B} SENDER: Fill in your personal information here.
• {C} RECEIVE: Please fill in “AIDEN” for the First Name, “EE DING” for the Middle Name and “CHONG” for the Surname.
• {D} The amount of money that is sufficient to pay to Sutera Sanctuary Lodges for your accommodation in Laban Rata, in words and figures.
• {E} This is the most important part: TEST QUESTION. PLEASE FILL IN “What is my email address?” AS THE QUESTION, AND PUT YOUR CORRESPONDENT EMAIL TO US AS THE ANSWER. It’s that easy. The optional message to us will be limited to 10 words only.
• {F} Don’t forget to sign and put the date.

When you finished with that, they will give you a reference number. Please email the reference number to us as soon as possible to speed up your request.
